- scleral buckle

- My 6-year-old is scheduled for an evaluation under anesthesia (UA) next week. The doctor needs to do UA because she can't get a good look at his eye otherwise. He has had surgeries w/Dr. Trese in his R eye for retinal detachment and had been seeing 20/400 in that eye, but at his visit yesterday he couldn't even recognize images at 20/600. Our doctor will most likely just get a good look at his eyes while UA, but she told us that if there has been more detachment in the R eye, she may want to do a scleral buckle. Has anybody had any experience with a scleral buckle? He has already had laser surgery and vitrectomy w silicone oil implant.

- Update: we had the UA and everything looked good. There was no need for the buckle. We had already decided prior to the UA that if a buckle was recommended we would probably hold off and go to Dr. Trese for a second opinion.

- Hi Sarah E,
I am very sorry about the news regarding your son’s vision and pray for the best. I have a buckle since in 1983. The eye with the buckle RE has lost vision very slowly over the years since 1983 so that now I only see light. I believe that the loss in vision is due to decreased blood flow to the retina causing the retina to slowly deteriorate and loose rods and cones. Over time that is very little left of the layers of the retina to see. This was the conversation I had with my retina specialist at the last visit.
The buckle has been very stable and there has been no need for additional repairs or work to fix the detachment or buckle. The buckle is the old way to take care of the detachment and in my experience was very successful. It seems to me from all the difficulty with the silicone process used now that the delicate retinas of those with CXLRS may not be best repaired with the new process (I have no evidence or medical data to prove this only the experiences reported on this board).
To be honest with you I see very little benefit from doctors looking closely at my retina. If the only outcome is their knowledge of my condition and there is no therapeutic benefit or improvement why go through the procedure. I would only do what is absolutely necessary and avoid risky trials. As I was told by a Dr. whose name is repeated often on this board, things can get much worse. Know the risks.
Let me know if you need other information, thanks and have a great New Year.
